湖南省和重庆市部分孕产妇HCV感染及影响因素分析Prevalence of hepatitis C virus among pregnant women from provincial maternal and child health hospitals in Hunan and Chongqing

摘要(Abstract):
目的 了解在湖南省妇幼保健院和重庆市妇幼保健院入院分娩的孕产妇人群HCV的感染情况及相关影响因素。方法 通过横断面调查的研究方法对2019年7月1日至2021年12月31日期间于湖南省妇幼保健院和重庆市妇幼保健院入院分娩的产妇进行流行病学调查。在研究地区妇幼保健机构内孕产保健科、产科、检验科和感染科等相关科室的病案管理信息系统收集调查相关信息。结果 在上述两家妇幼保健院共收集符合条件的研究对象84 814名,其中65人感染HCV,感染率为0.08%(95%CI:0.06%~0.10%),不同年龄组间HCV抗体阳性率差异有统计学意义(χ2=38.10,P<0.001),且随年龄增加感染风险增加(Z=5.35,P<0.001)。不同梅毒合并感染状态(χ2=38.10,P<0.001),且随年龄增加感染风险增加(Z=5.35,P<0.001)。不同梅毒合并感染状态(χ2=111.37,P<0.001)、前置胎盘(Fisher检验P<0.001)、婴儿出生体重(F=4.15,P=0.04)与HCV抗体阳性率相关。多因素分析发现,与小于30岁的孕产妇相比,年龄超过35岁的孕产妇感染HCV的风险更高(OR=4.76,95%CI:2.51~9.05),与未感染梅毒的孕产妇相比,感染梅毒者检出HCV的风险更高(OR=14.68,95%CI:6.59~32.68)。结论 在湖南省妇幼保健院和重庆市妇幼保健院入院分娩的孕产妇HCV感染率处于较低水平,但仍需要重视孕产妇HCV感染及其相关因素,关注梅毒等性传播疾病的合并感染。
